As x64 VST comes only with Nebula server, you must have Nebula server installed to have preview working.
I guess 32 bit version will not come very soon, because there is a lot of weird stuff to be done to support Carbon based 32 bit plugins inside Cocoa based Nebula Setups. Anyway i am rather deep into this stuff right now, so anything may happen =:)
Again, make sure you have x64 VST Nebula plugin available and check program's x64 settings as well. If wrong plugin is used for preview, an empty window will be shown.

Yes, there is. It is called default.nmdictionary.
On windows it is located in the same folder as program's executable. On osx it is stored inside app (open package) file - can't remember the exact location right now, it shouldn't be hard to find it.
It's an editable text file in xml format. You can find category descriptions inside - structure is rather straightforward. Also, Nebula Setups uses this file to retrieve available parameters for tweaks list.
